A 61 years old man presents with itching all over the body

 I am R yaswanth  Reddy  , 3rd Sem Medical Student.This is an online E log book to discuss our patient's de-identified health data shared after taking his/her/guardian's signed informed consent.

I've been given this case to solve in an attempt to understand the topic of "patient clinical data analysis" to develop my competency in reading and comprehending clinical data including history, clinical findings, investigations and come up with a diagnosis and treatment plan. 

Chief complaints

-Itching all over skin since 4 years

- SOB during cold climates 

Addiction

Alcoholic since 15 years stopped 6 years ago

Clinical findings

- Scaly erythematous plaques noted over upper extremities and lower extremities  

- Auspitz sign ( bleeding points when scale is lifted from plaque)

- plaque covered by silver white scale

 

Provisional diagnosis : chronic plaque psoriasis
